First Appointment
Please arrive at least 30 minutes prior to your first appointment.The following should be brought with you to your first appointment
- Completed New Patient Forms
- Patient Registration Form
- Health History Form
- Applicable insurance copay, deductible or payment in full if you are not covered by insurance. Payment depends upon your insurance type and plan. Payments are collected when you check in.
- Valid photo ID
- Medicines that you are currently taking. [Please bring your medication in their actual bottles so your medical care provider can determine exactly a medications and dosages you are on.]
- Medical Records: If you are referred from other physicians or treated by other physicians please complete authorization form. You can find this form here.
Subsequent Appointments
Please arrive at least 30 minutes prior to your first appointment.The following should be brought with you to every appointment.
- Current insurance card(s)
- Valid photo ID
- Current Medication list which includes the following information (name of medicine, number of times a day you take the medication, how many mg etc) OR bring all your medication bottles including over the counter medications and supplements such as aspirin, vitamins etc.
- Applicable insurance copay, deductible or payment in full if you are not covered by insurance. Payment depends upon your insurance type and plan. Payments are collected when you check in.
