5. Does the Health and Wellness Clinic cost me any thing?
Office visits and Labwork are free for:
  • Full- time employees.
  • Retirees enrolled in City's health plan
  • Employee and retiree spouses enrolled in the City's health plan
 
6. Can I go to the Clinic if I am not enrolled in City insurance
Yes, If you are a full-time employee. Retirees, retiree and employee spouses must be enrolled in the City's health plan ato utilize the clinic.
7. Can I take my dependant children to clinic?
Clinic will not provide services for dependant children of employee or retirees.
8. How can I get started?
The Health and Wellness Clinic is open Tuesday, Wednesdays, and Fridays from 8 am to 4:30 PM. 12:00PM to 1:PM is lunch break. You can make appointment by calling (281) 425-3868 or (281)425-3932. Walk-Ins are also accepted.
9. What Happens when I call?
Jacinto Medical Group's appointment desk will answer your call. Identify yourself as full -time employee, retiree who is enrolled in City's health plan or an employee of retiree spouse enrolled in the City's health plan. Then ask for City of Baytown Employee and Wellness Clinic Appointment.
10. What happens at my appointment
The clinic staff will provide you with treatment, If necessary, personalized health care education and ideas on healthier living.
11. Who will provide service to me?
A Physician Assistant will give you treatment and advice.
12. Will I be able to get a prescription?
Yes, the Physician Assistant can write prescriptions. Prescriptions can be filled at any pharmacy and you are responsible for the cost or co-payment. Please see your treating Physician for maintenance prescriptions.
13. Can I get my annual well woman/man exam at the City of Baytown Employee and Wellness Clinic?
No. However, information and education is available on women's/ Men's health issues.
14. Are services confidential?
Yes, All record, including medical information and evaluation, are kept confidential in accordance with federal and State laws. All files are property of Jacinto Medical Group, who will keep and maintain them at all times. The City will not be sent copies of your medical files.
15. What should I do in emergency?
If your situation is serious of life threatening, you should go directly to hospital emergency room or call 911 for assistance.
