Selecting a Primary Care Physician Patients on an HMO plan must select a Jacinto Medical Group primary care physician with their health insurance provider in order to access all of the services available at Jacinto Medical Group. Primary care physicians are available throughout JMG locations in Baytown and Laporte. Please note that some physicians may not be accepting new patients at all times.
